The State of CGT 2026

The cell and gene therapy industry is entering a defining new phase. Scientific breakthroughs are becoming commercial realities, but the challenges of scaling, financing and delivering these therapies are bringing a new set of priorities into focus.

Nearly a decade after the first CAR-T therapy was approved, the conversation is shifting from what advanced therapies can achieve scientifically to what it will take to make them commercially sustainable and accessible to patients. Investment pressures, manufacturing complexity, evolving delivery technologies and global regulatory requirements are reshaping how the industry approaches its next stage of growth.

Yet the opportunity remains significant. With a growing pipeline of clinical programs, an expanding portfolio of approved therapies and increasing commercial revenues, the sector is demonstrating its potential. The question now is how the industry can translate that momentum into sustainable growth, overcome barriers to patient access and deliver advanced therapies at scale.

The scale of activity across the sector demonstrates just how far the field has progressed. More than 2,000 CGT clinical trials are currently underway globally, while 122 CGTs had been approved as of April 2026, comprising 46 gene therapies, including genetically modified cell therapies, and 76 non-genetically modified cell therapies. Four CGTs are now generating more than $1 billion in annual revenue, underlining the sector's growing commercial maturity.
